Empire Plan
The Empire Plan provides medical and
hospital insurance coverage to school district employees and their
dependents. For a searchable list of participating providers

The Empire Plan offers a personalized web site for subscribers at
www.myuhc.com. Here you can review
your claims history and print out copies of your Explanation of Benefits
forms. Registration is required. When registering enter #30500 in the
Group/Account field.
Medco by Mail (mail order prescriptions)
Medco by
Mail is now the Empire Plan's mail order prescription service for new
prescriptions as well as for renewals of prescriptions placed with
Caremark.

Fitzharris Benefits Administrators reviews and processes
dental claims for The Long Beach Public Schools. The Long Beach Public
Schools is the actual insurance provider.

on the status of a claim call 777-2244 or 1-800-321-1336. Be sure to have the
following information ready when calling:
Employee's name and social security number
Patient's name
Date of service

exceeding $350.00" is not applicable to our plan.
Flex Plan
The Flex Plan (a.k.a. Cafeteria Plan) offers our members
significant tax savings on their excess medical and child/elder care
expenses. Claims for expenses incurred in 2004 should be sent to:

Preferred maintains an active website at www.thepreferredgroup.com.
Here you can download a claim form and find information on what expenses
are eligible for reimbursement. Registration
is now required to access the site.
403(b) Tax-deferred Annuities
The OMNI Group, the
company that the district has employed to administer its 403(b) plan,
maintains an active web site at
http://www.omni403b.com. Once there click on “Clients”/ “Client Home
Pages” and then select “Long Beach City SD (NY)” from the list of
organization’s home pages. Here you can download forms, find answers to
frequently asked questions and review the list of participating
providers.
EAP
The Employee Assistance Program is available to assist our
members with just about any problem that may arise. It is open 24 hours
